If you or a family member needs hospital care, the claim process can feel confusing. Two common ways a health insurance claim may be handled are cashless and reimbursement. The exact process depends on your insurer and the terms of your policy.
What is a cashless claim?
With a cashless claim, you ask the insurer or its claims administrator to approve eligible hospital expenses at a hospital in its network. The hospital usually sends a request for approval. Cashless treatment is subject to the insurer’s approval and policy terms.
“Cashless” does not always mean every bill item is covered. Depending on your policy, you may still need to pay for items or amounts that are not covered, any co-payment, or other deductions.
What is a reimbursement claim?
With reimbursement, you pay the hospital first and then submit a claim to the insurer or claims administrator for review. The insurer will tell you which forms and documents are needed, how to submit them, and the applicable time limits. Keep the insurer’s instructions and your claim reference number.
Call the number on your policy or health card, or use the insurer’s official app or website. Ask about the correct process and deadlines for your specific policy.
Before a planned hospital admission
- Check whether the policy is active and the hospital is in the insurer’s current network.
- Ask about waiting periods, exclusions, room limits, co-payment, and other policy conditions that may apply.
- Ask the hospital insurance desk how to start pre-authorisation and what documents are required.
- Get the insurer’s process and any applicable time limits directly from the insurer or claims administrator.
During an emergency
Get the medical care needed first. As soon as practical, contact the insurer or ask the hospital insurance desk to contact it. If cashless treatment is unavailable or not approved, ask the insurer what to do next and whether you can submit a reimbursement claim under your policy. Keep a note of who you spoke with and your reference number.
Keep these records
The insurer will confirm the documents needed for your claim. These may include claim forms, discharge summary, itemised hospital bills, payment receipts, prescriptions, and test reports. Keep copies of everything you submit, and use only the insurer’s official channel for medical records and identity documents.
Does pre-authorisation guarantee payment?
No. A pre-authorisation or cashless approval is not necessarily a promise to pay every item on the final bill. The insurer reviews the claim against the policy terms and may apply limits, exclusions, or deductions. Read the policy documents and ask the insurer if any part of the process is unclear.
If you need help
Start with the insurer’s official claims or grievance channel and keep the complaint reference number. If the issue remains unresolved, you can learn about escalation through IRDAI’s Bima Bharosa portal and the Insurance Ombudsman.
